Forms and documents needed to enroll your student. Complete the online registration form first, then print and complete the Medical Release and Handbook Agreement forms and bring them to orientation.

- 2026–2027 Registration Form (online form) — Required for all students.
- 2026–2027 Participant Handbook (PDF) — Program policies, expectations, tuition, dress code, and rehearsal details. Every family reads it before signing the agreement below.
- Handbook Agreement Form (PDF) — Signed by parents and students; print and bring to orientation.
- Medical Release Form (PDF) — Required for every student; print, sign, and bring to orientation.
- Band Supply List (PDF) — Instrument-specific supplies, required method books, and the music stand specification.
